Yeah, it does appear that he was knocked off of the Who's Online list at some point.

So, I skimmed the SMF source code to see what might have gone wrong -- I was a little surprised at how the Who's Online list actually works.  There is a table that holds the entire list.  When the table is queried, there is no where clause, it always grabs the entire table.  This means it is up to the software to maintain the list manually through updating and deleting entries from it from time to time.

(I would have guessed that the users table would have a "last time active" column and that the Who's Online list would query the user table for users with a "last active time" less than 10 or 15 minutes).

I haven't tracked through all of the cases when SMF will update or delete rows from the Who's Online table, but as far as I can tell, Kosmikat (likely without realizing it) did something that either caused the expected update or delete to fail, or found some scenario where SMF is not doing an update or delete to the table when it should be.

Did you google the guy?  His portfolio is pretty impressive.  He's done some cover artwork for Nintendo Gamer magazine (and its predecessors).

Hmm...I have his DB entry this time.  He does seem to actually be active, since the DB entry is getting updated, but however he is accessing the forum it does not update his "Last Active" time or change his "Action".  There is a reference to "mobiquo" in the DB entry, so maybe he's using the Tapatalk API in some unusual way.  What's odd is that his user agent string shows that he uses Chrome and not Tapatalk.

There apparently is a "Tapatalk Notifier" Chrome extension that monitors forums for updates.  Maybe that's what's causing it?  I suppose I could test it out later.
