I could take a look in my parts bin and see if I have one left. I may have sold the last board I have though, so I'll have to check.

Have you checked the fuse or bypassed it?

I've only seen it with my own programmed FDS programs. :-[
Seems like the disk is accepted since it doesn't throw any errors but the files are never loaded?
