It's Clu Clu Land. The FDS version could only be bought via a disk writer kiosk.
That's the thing, I've never seen it had the little art picture inside the plastic case (Meaning it's a store release, I thought this was a disk-kiosk exclusive)

Its included with a new disk purchase from the kiosk
A lot of people also print their own sleeves and stickers.
I remember someone on a thread said that Clu Clu Land did have a standard release in 1992 that had the obi.
Like Ross said you got obi, stickers and (fold-out) manual for disk writer versions as well. They were supposedly provided by the store assistant that wrote the disk for you.
Clu Clu Land FDS never had a standard release and Disk Writer games did not include obis. You usually got stickers and a fold up manual but no sleeve. That Clu Clu Land sleeve is certainly a fan-made custom print, I made a whole set of them for the six disk-writer-only Nintendo published games myself.
