I've come across this disk many times, but I haven't been able to find the name of the publisher or developer, which is in Japanese on the game. Anyone have a clue?

This would be Kawada. 河田

There's a small chance that the maker's name is read "Kawata" (with a hard t instead of d... reading can differ from region to region).
