It depends, if you go to a proffesional studio, then no (well there are places on your body where it does hurt, but you probably won't have your tatoo there ). But if you go to somebody who hasn't a profesional tool to do it, then yes, it hurts, but nothing a man can't withstand
Just don't drink alcohol before it. It will bleed more.

Hmm never heard of it (I have some tatooist friends and they never use it), maybe it can be done somewhere... But don't be afraid of it, it's fun and you'll remember it for the rest of your life

If you plan to live in Japan, a tattoo is not a great idea.  People will think you're a criminal, even if it's not a Chinese or Japanese style tattoo.  You will also never be allowed into a Japanese bath ever gain.  Should that matter.

Wait when i get on a PC ( i am not home right now and typing from my phone) i will take a picture of my hand and make a mockup of my future tatoo... if you want i can make mockups for you too i am great with that nobody can guess is it a fake or real tatoo

Of course getting a tattoo hurts.  How much it hurts depends on your pain tolerance.  Some people puke and pass out from the pain, others just shrug it off.

Anyway my favourite tattoos are something that sends a visual meaning.  I actually think chinese characters can work that way since they're kind of like pictographs.  When I see someone with a tattoo of an english sentence or written in a language that has phonetic based writing I really don't see the point.  I also think someone should find an image that strongly portrays the message behind the image.  That way when people look at it they'll be like "yeah I get it" without you having to really explain.

Really though anything you stick onto your body you should pick for yourself.

You might be lucky in some shady bath houses run by people who are near to the Yakuza. But then I wouldn't really want to bathe amongst Yakuza guys...
